Inevitably there are concerns when running a business as to whether all that is necessary to meet your obligations is being done. Even when a business is being run diligently the very fact of being close to an activity in a familiar environment can lead to a familiarity that results in simple matters being overlooked.  

How often have you got into the habit of stepping over an obstruction, just because it has always been there or fail to see an acknowledged risk because of being unaware of its potential?
Sometimes it needs a fresh pair of eyes to see the obvious or not so obvious!
Running a successful company is a time consuming activity, there are many calls on a manager's or executive's time.  Sometimes issues, often important issues, (and the safety of the workforce must fall into this category)  get pushed down the agenda and some impetus is needed to raise the profile again.
